GCH begins health initiative

GARDEN CITY — Garden City Hospital offers the new community outreach health initiative LinkUp with a Health Coach three Wednesdays a month.

LinkUp programs are available from 9:30 a.m. to 1 p.m. the third Wednesday of the month at First United Methodist Churcyh, 6443 Merriman Road, and 10 a.m. to 1 p.m. the second and fourth Wednesdays of the month at Focus: HOPE, 759 Inkster Road, Inkster.

The free program was created to address the growing health care needs within the community for people age 18 and up who are low income, uninsured or underinsured. The goal is to provide prevention, treatment and the coordination of services that address heart disease and diabetes among the region through the use of the hospital’s mobile unit, called the Health Coach.

Services offered include health screening and assessments, health coaching, diabetes case management, community health resource referrals, health information and patient care followup.
